Marion Public Health’s Office of Vital Statistics houses certificates of birth and death occurring in Marion County from 1908 to the present.  We can also produce birth certificates from other counties in Ohio.  Currently, death certificates are only issued by the county where the death occurred.

The fee for birth and death records is $25.00 each. Marion Public Health will accept cash, money order, or cashier’s check made payable to Marion Public Health.  We also accept debit and credit cards. The credit card processing agent will apply a minimum fee of $2.50.

Marion Public Health will not accept personal checks for the purchase of a birth or death certificate.

For your convenience, you may obtain a birth or death record online through an independent company that we have partnered with to better serve you. To process your request through our office:

Please download the birth and death certificate application.  Once completed, you can bring in the application to us, along with the fee, or come to our office for processing. Please do not mail in your application and cash or personal checks – we are not able to process mailed-in certificate requests.
