The Vital Statistics office prepares paternity affidavits, death and birth affidavits, burial permits as well as registers all death and home birth events occurring in Marion County. Marion Public Health’s Office of Vital Statistics houses certificates of birth from 1908 and death’s occurring in Marion County from 1954 to the present.

We can also produce birth certificates from any county in Ohio.

Currently death certificates are only issued in the county where the death occurred.

No documents are needed when you arrive to get your birth certificate. Please see Death certificate section for more information on what documents are needed for death certificates.

The Fee for the Birth and Death Certificates are $25.00 each Marion Public Health will accept cash, credit or debit card, money order or cashier’s check. Please note if using a credit card an additional $2.50 charge will be applied. No personal Checks will be accepted.

 

Birth Certificates

Marion Public Health is a participant in statewide issuance. We can issue a birth certificate to anyone born in any county in Ohio. Information necessary when making a request includes:

  • Name at birth
  • Date of birth
  • If applicable, father’s name
  • Maiden name of the mother

Walk-In Orders

The Vital Statistics office is located at 181 S Main St Walk-in hours are 8 a.m. to 4:30 p.m. Your certificate is prepared while you wait.

The fee for birth is $25.00 each. Marion Public Health will accept cash, money order, or a cashier’s check made payable to Marion Public Health.  We also accept debit and credit cards. The credit/debit card processing agent will apply a minimum fee of $2.50 when using a card.

Please download the birth and death certificate application.  Once completed, you can bring in the application to us, along with the fee, or come to our office for processing.

Marion Public Health will not accept personal checks for the purchase of a birth or death certificate.

No identification documents are needed to obtain your Birth Certificate .Ohio is an open record state any Individual capable of providing accurate birth information can obtain the birth record.

All Marion Public Health issued Birth Certificates meet the requirements for passports or any other document requiring proof of a birth certificate. If you need to fold your birth certificate, we ask that you do not fold across the seal.

Orders can be placed online using the VitalChek website. VitalChek adds a service fee for processing this order and sends it to us to prepare.  Expedited shipping is offered.

Home Birth Information

Home births are registered by the Health Department.  Guidance on preparation of the appropriate forms is available by calling the Vital Statistics office.  The forms may be downloaded and completed and brought to the vital statistics office where filing will be completed.

Death Certificates

Marion Public Health registers all deaths which occurred in Marion County Only. Death certificate requests in which the death occurred in another county must be obtained through the county of death.

All death certificates will be issued without a social security number, unless identification is provided confirming you are an authorized requestor.

Funeral Home Services

The Vital Statistics Office will register death certificates presented in person or through the mail or by filing electronically. We require a completed Birth/Death Application to issue certified death certificates and burial permits.

Mail Filing

Certificates to be registered through the mail will need to provide the application form along with the signed paper death certificate in an unfolded envelope, sent to the following address:

Please include a self-addressed stamped envelope.

Electronic filing

Electronic filing can be completed by emailing a completed and signed paper death certificate. It can be emailed with a request for approval at Certificates@marionpublichealth.org we will review the certificate and accept or reject the filing within one business day. Please use a Request to E-File cover sheet when emailing a death record to a local office. Please register your Funeral Home on vitalchek.com to purchase copies with a credit card.

Burial Permits
Permits may be obtained from the Marion Public Health Department Monday – Friday, from 8:00 am to 4:30 pm. Burial Permits must be paid for in advance of approval unless otherwise approved by the local vitals office. Out of town funeral homes are able to use their local vitals office for approval. Additionally, sub-registrars are empowered to issue burial permits on behalf of Marion Public Health outside of the normal business hours:
