Now that we have published the 2015 Community Health Assessment, we are once again asking for the public’s participation as we develop the plan to address the 5 priority health issues identified in the assessment.
We will post links to surveys to gather your thoughts on:
- Broad Goals within each of the 5 Priorities
- Thoughts on “root causes” of the problems they address
- Feedback on Objectives and Strategies
